Three major types of benefits,
medical benefits to treat your injury, lost wages and permanent disability
benefits, and permanent total
disability benefits.
An employer shall report a work-related injury or illness on the employers first
report of injury.The employer shall submit the employers first report of injury
form to its insurance carrier within the 10-day period.
When the final reports are completed, reports shall accompany the admission
including any evaluation record associated with an impairment rating. The
admission shall specify and describe the insurance carriers position on the
medical benefits after MMIs the admission shall make specific reference to
the medical report by listing the physician's name and the date of the report.
